Ghrelin
Ghrelin is a peptide hormone initially identified in gastric tissue as an endogenous ligand of the growth hormone secretagogue receptor. Characterization requires distinguishing its acylated form from preparations without that modification.

Mechanism described in the literature

A natural ligand for a known receptor
The 1999 investigation began with a receptor known for responding to synthetic secretagogues. Identifying a gastric ligand connected that experimental pharmacology to an endogenous molecule. This history distinguishes hormone discovery from the design of compounds sharing its receptor system.
GOAT and ghrelin processing
The 2008 study identified GOAT and examined its role in ghrelin modification. In mouse models, disruption of the corresponding gene eliminated detectable circulating acylated ghrelin. This separates production of the peptide from a later molecular modification step.
Which form the assay recognizes
A measurement called total ghrelin should not automatically be interpreted as the amount of acylated ghrelin. The analytical question is whether the method differentiates the forms. Nor should unacylated ghrelin be described as lacking all biological activity: the cited experiments delimit a specific function, not every possible interaction.
